Wellness Centre Waiver and Release of Liability

  • ACKNOWLEDGEMENT OF RISKS I, [Full Name], acknowledge that my participation in activities at Wellness Social Club, including but not limited to the use of saunas, pools, red light beds, oxygen chambers, yoga classes, and other wellness services, involves certain inherent risks. These risks may include, but are not limited to, physical injury, burns, dehydration, fainting, allergic reactions, muscle strains, sprains, cardiovascular issues, or other health-related complications. I understand that these risks may arise from my own actions, the actions of others, or the condition of the premises, equipment, or facilities. I confirm that I am voluntarily participating in these activities at my own risk.

  1. RELEASE AND WAIVER OF LIABILITY To the maximum extent permitted by law, I hereby release, discharge, and hold harmless Wellness Social Club, its owners, employees, contractors, instructors, and agents from any and all liability, claims, demands, or causes of action that may arise from injury, illness, property damage, or any other loss resulting from my participation in wellness activities or use of the facilities. I understand that Wellness Social Club is not responsible for any injuries or health issues sustained while using its services, whether caused by negligence or otherwise, including but not limited to improper use of equipment, incorrect participation in activities, or pre-existing medical conditions.

  2. MEDICAL DISCLAIMER I confirm that I am in good health and have no medical condition that would prevent me from safely participating in the wellness activities offered by Wellness Social Club, including saunas, pools, red light beds, oxygen chambers, or yoga classes. If I have any concerns about my health, I will seek medical advice before engaging in these activities. I acknowledge that Wellness Social Club does not provide medical assistance, diagnosis, or treatment, and I am solely responsible for managing my health and any medical needs that may arise.

  3. USE OF FACILITIES AND EQUIPMENT I agree to use the Wellness Centre’s facilities, equipment, and services responsibly and in accordance with all instructions, guidelines, and safety protocols provided. I will not engage in reckless or hazardous behaviour that could endanger myself or others. I understand that any damage caused to equipment, saunas or facilities due to misuse may result in financial liability. I agree to follow all safety rules, centre policies, and instructions given by staff or instructors.

  4. PERSONAL PROPERTY I understand that Wellness Social Club is not responsible for any lost, stolen, or damaged personal belongings brought onto the premises. I accept full responsibility for securing my personal items while using the facilities.

  5. GOVERNING LAW This waiver is governed by the laws of Victoria, Australia, and any disputes arising from this agreement will be resolved in accordance with Australian law.

  6. ACKNOWLEDGEMENT & SIGNATURE I confirm that I have read and understood this waiver in full. I voluntarily agree to the terms and conditions and acknowledge that I am giving up certain legal rights by signing this document.
